If you've lived with ADHD for years, you may know your own patterns pretty well. You know where you tend to lose focus, what helps you stay organized, and which routines keep everything moving.
Then the menopause transition begins — and suddenly some of those systems may not seem to work quite as well.
You may lose your train of thought more easily. Familiar work can require more concentration. Finding the right word takes longer. Multitasking becomes exhausting. Small interruptions can completely derail what you were doing.
And that raises an interesting question:
Is ADHD actually changing during the menopause transition?
There May Be More Than One Thing Happening
Perimenopause itself can bring difficulty concentrating, forgetfulness, sleep disruption and emotional changes.
The problem is that several of those experiences also overlap with ADHD.
So for a woman who already has ADHD, it may not be as simple as saying, “My ADHD suddenly got worse.” There may be several things happening at once.
Poor sleep can make attention harder. Night sweats and hot flashes can fragment sleep. Stress can further strain executive function. And changing hormone levels may add another layer.
The Estrogen–Brain Connection
Estrogen isn't only a reproductive hormone.
It also interacts with neurotransmitter systems in the brain, including dopamine — a chemical messenger involved in attention, motivation, reward and executive function.
That provides a plausible biological connection between hormonal fluctuations during the menopause transition and changes in how some women experience attention, mood and cognitive function.
But this is also an area where the science is still developing.
What Does the Research Actually Show?
One recent population-based cohort included 535 women with ADHD and 4,857 women without ADHD.
Severe perimenopausal symptoms were reported in 54.2% of the women with ADHD compared with 30.1% of those without ADHD.
That's an intriguing difference.
But the story isn't settled.
Another recent study did not find significantly worse menopausal complaints simply based on whether a woman had an ADHD diagnosis.
That distinction matters.
The evidence does not currently justify saying that menopause automatically makes ADHD worse. Instead, it suggests an important intersection between ADHD, hormones, cognition, sleep and menopause that researchers are only beginning to understand.
Three Compounds Worth Knowing About
Supplements are not treatments for ADHD, but several compounds have been studied for aspects of cognitive function, attention and stress.
Citicoline
Also known as CDP-choline, citicoline provides compounds used in the synthesis of phosphatidylcholine, an important component of neuronal cell membranes.
It has also been studied in connection with neurotransmitter function, attention and cognitive performance.
L-Theanine
L-theanine is an amino acid naturally found in tea.
It's often thought of simply as a relaxation ingredient, but research has also explored its relationship with attention, stress response and cognitive performance.
That makes it particularly interesting when someone describes feeling simultaneously “wired” and unable to focus.
Magnesium L-Threonate
Magnesium L-threonate is different from simply talking about magnesium in general.
This particular form was developed with increasing brain magnesium in mind and has been studied in relation to aspects of cognitive health.
It isn't an ADHD treatment, but it represents an interesting area of nutritional research involving the brain and cognition.
The Bigger Point
If ADHD symptoms suddenly seem worse during the menopause transition, don't automatically assume you're simply becoming less capable.
And don't automatically assume every new cognitive symptom is ADHD.
Sleep problems, hormonal changes, medications, mood disorders, thyroid issues, nutrient deficiencies and other health conditions can produce overlapping symptoms.
There may be multiple factors worth discussing with your healthcare professional — particularly when changes in attention, memory, mood or everyday functioning are significant.
The larger story is one women's health research is only beginning to explore properly:
Menopause doesn't happen independently of the brain conditions and cognitive patterns a woman already has.
For women with ADHD, that intersection deserves considerably more attention.
